How to locate a specific publication/form
Our search page offers many options for finding current and past ODHS/OHA publications or forms. When using the search page:
In the publication/form number (Find a publication or form) field, do not start with letters.
If you don’t have a publication/form number, you can use the keyword search to help locate it. Enter one or two words from the name of the publication/form, such as "child care".
You can narrow your search by entering information into more than one field, or using more words in the 'title contains' field. Enter less information to find more publications/forms.
Remember to enter dates in the format of MM/DD/YYYY. For example, January 15, 2004 would be entered as 01/15/2004.
